• Hepatitis Vaccine (if they've received it) 

• $10.00 Cash for Criminal Background Check

C.N.A. / H.H.A. CNA Certification / HHA Certificate

• 2 WRITTEN References from R.N./L.P.N. with Phone #'s

• Current Physical (w/in on year)

• Current TB (w/in one year)

• Social Security Card / Birth Certificate

• Driver's Licenses / Photo ID

• Child Abuse Clearance
