What is a Registered Nurse Geriatric Care Manager?

You maintain control, we absorb the stress, and you save time, frustration, and money. 

Our involvement is boundless. We will evaluate your unique needs and put a customized plan of care in place. We coordinate in-home/facility based care. We are there for crisis intervention, the overseeing of medication and medical management, or simply the coordination of daily needs.

Fees are charged on an hourly basis to include site visits, telephone follow-up, and coordination. Most clients save time, frustration, and money by using a GCM, who is specially trained to navigate the multi-leveled long term care environment.
